How to Make a Google Form: Let's Do This!

Step 1: Get to Google Forms

  1. Open your browser and go to Google Forms.

Step 3: Give it a Name!

Step 4: Add Your Questions

  1. Click on the first question box.
  2. Type your question.
  3. Pick the type of question. Google Forms has lots!
  • Short Answer: For names, emails.
  • Paragraph: For longer answers.
  • Multiple Choice: Pick one answer.
  • Checkboxes: Pick many answers.
  • Dropdown: Like multiple choice, but in a dropdown.
  • Linear Scale: Rate something from 1 to 5.
  • Multiple Choice Grid: Rate a bunch of things on the same scale.
  • Checkbox Grid: Pick many options for many things.
  • Date: Pick a date.
  • Time: Pick a time.
  • File Upload: Let people upload files. Important: needs Google Drive
  • Add the answer choices if it's multiple choice or checkboxes.
  • Want a question to be required? Turn on the "Required" switch.
  • Step 5: Make Questions Look Great

    You can add stuff to make your questions better:

    • Pictures or Videos: Add these to explain things or just make it look nicer.
    • Descriptions: Explain the question better.
    • Sections: Break your form into parts.
    • Go to Section Based on Answer: This is cool! Send people to different sections based on what they answer. Like a "Choose Your Own Adventure" form.
    • Shuffle Option Order: For quizzes, mix up the answers so people can't cheat.

    Step 6: Add More Questions!

    1. Click the "+" button to add another question.
    2. Do steps 4 and 5 again for each question.

    Step 7: Make it Look Good!

    1. Click the "Customize theme" icon (the paint palette) at the top.
    2. Pick a header image, colors, and fonts.
    3. You can upload your own image or use one from Google.

    Step 8: Form Settings - Important!

    1. Click the "Settings" tab at the top.
    2. General Settings:
      • Collect email addresses: Should Google grab their email?
      • Limit to 1 response: One answer per person?
      • Edit after submit: Can they change their answer later?
      • See summary charts and text responses: Can they see what everyone else said?
    3. Presentation Settings:
      • Show progress bar: Shows how much they have left.
      • Shuffle question order: Mix up the questions.
      • Show link to submit another response: Do you want them to fill it out again?
      • Confirmation message: Change the message they see after they're done. "Thanks!" or something more fun.
    4. Quizzes Settings:
      • Make this a quiz: Turn it into a quiz!
      • Release grade: When do they see their grade? Right away? Later?
      • Respondent can see: What can they see after the quiz? Right answers? Points?

    Step 9: Preview - Check it Out!

    1. Click the "Preview" icon (the eye) at the top.
    2. Fill out the form yourself to test it.

    Step 10: Share It!

    1. Click the "Send" button at the top.
    2. Pick how to share:
    • Email: Type in email addresses.
    • Link: Copy the link and send it anywhere. Shorten the link - looks better!
    • Embed: Put the form right on your website.

  • Seeing the Answers

    People are answering! Now what? All the answers go into a Google Sheet automatically.

    In Google Forms

    1. Open your Google Form.
    2. Click the "Responses" tab at the top.
    3. See the charts and graphs!
